Fear Avoidance Belief Questionnaire (FABQ)

Description:

A self-report questionnaire. It evaluates the perceptions and impact of chronic back pain on patients with regard to their beliefs, fears and fear avoidance behaviour.

Two fields are evaluated : physical state and work.

This scale is composed of 16 questions divided into two fields: FABQ Physical Activity (5 questions) and FABQ Work (11 questions).

For each question the patient must give a score between 0 and 6 where 0 represents "completely disagree" and 6 represents "completely agree".

The score is then calculated by summing the following items from the two fields: Scale 1 : beliefs regarding work (6 + 7 + 9 + 10 + 11 + 12 + 15 , score from 0 to 42) and Scale 2 : beliefs regarding physical activity (2 + 3 + 4 + 5 , score from 0 to 24).
